Federal Healthcare Changes: How ALS Northwest Will Support the ALS Community

On July 4, 2025, H.R.1 became law, bringing significant changes to federal healthcare programs. The legislation modifies federal spending for Medicaid, Medicare, and the Affordable Care Act (ACA) over the next decade. These changes will affect how millions of Americans — including thousands of people living with ALS and their caregivers — access essential care and services. 

What’s Changing 

The new law includes: 

  • A $911 billion reduction in federal Medicaid spending over 10 years 
  • New requirements for certain Medicaid recipients beginning in December 2026, including reporting work or community activities 
  • Changes to eligibility verification processes, with reviews every six months for some recipients 
  • Modifications to how states fund their share of Medicaid costs 
  • Potential impacts on Medicare coverage for some beneficiaries 
  • A $50 billion Rural Health Transformation Program to support rural healthcare providers 

According to the Congressional Budget Office, an estimated 10.9 million people may lose health coverage by 2034 as these provisions are implemented.
