I hereby grant permission to ALS Northwest to use my likeness in video and/or photographic materials for promotional and educational purposes. I understand and agree to the following terms:

1. Release of Likeness: I authorize ALS Northwest to use, reproduce, and distribute images and/or videos of myself taken during ALS Northwest events, programs, or activities.

2. Purpose of Use: I understand that my likeness may be used for promotional materials, including but not limited to, brochures, newsletters, websites, social media, and other marketing materials.

3. No Compensation: I waive any right to inspect or approve the finished product wherein my likeness appears. I understand that I will not receive any financial compensation for the use of my likeness.

4. Release: I release ALS Northwest, its employees, and any third parties involved in the creation or publication of marketing materials, from liability for any claims by me or any third party in connection with my participation or the use of my likeness.

5. Duration of Consent: This release shall remain valid indefinitely unless otherwise revoked by me in writing.

6. Revocation of Consent: I understand that I may revoke this consent at any time by providing written notice to ALS Northwest. However, any materials already produced or distributed prior to the revocation shall remain in circulation.
